Derbyshire County Council has a £7.39 million kitty to bring the service to areas that haven't yet got access to it.
Bosses want as much feedback as possible so they can gage public opinion on both business and recreational internet use.
The Conservative-led coalition thinks the money will benefit more than 188,000 homes across the county that currently have a poor broadband service.
The survey can ben completed via the Derbyshire County Council's website at www.derbyshire.gov.uk
